Hydrospa spa control panel 6103 by Spa Builders with graphic overlay included.  Also includes cable and plug.

6103 includes the overlay

Overlay with 4 buttons showing pump, light, temp up and temp down included.  Overlay may ship with more indicator spots as the control panel underneath has a complete line of indicator lights anyways.  Unused indicator lights by the control system will not illuminate.
